Saving Health
If you have diabetes and live in a participating county, the West Virginia Public Employees Insurance Agency is offering a new program to improve your health and save you money.
You can expect to receive the best diabetes care available as your pharmacist will work with you to ensure that you: a) are current on all recommended testing and treatment; b) know how to properly take prescribed medicines; and c) have a physical activity and nutrition plan that will assist you in achieving optimal health.
How it works
If you or your dependents are in PEIA's Preferred Provider Benefit Plan and have diabetes, you can sign up to participate in Face To Face.
You will select a participating pharmacist in your area. During your first visit to your pharmacist, you will be required to sign a contract stating that you agree to make regular visits to this pharmacist for counseling services about your diabetes. You will also be expected to follow through on your agreed plan of care.
Saving Money
PEIA will waive copayments for diabetes-related prescriptions and labs if you participate in the program and keep your Face To Face appointments while following your plan of care. This could save you hundreds of dollars.

Public Enrollment
To enroll in the Face to Face program, please fill out our enrollment form. Before filling out the form, you will need to have available your Social Security Number, the primary cardholder's PEIA ID number, and your physician's contact information. If you are an Avantra member or an Access WV member, you will need to have those ID numbers as well.
